Botulinum Toxin Therapy for Vaginismus: Treatment That Restores Intimacy

Vaginismus is an involuntary spasm of the pelvic floor muscles that makes intercourse — and even a gynecological exam — painful or impossible. Botulinum toxin therapy is a recognized method for breaking the cycle of "pain → fear → spasm."

How the method works

Botulinum toxin injected into the spastic muscles temporarily reduces their excessive tone. This eliminates the painful spasm and gives a woman — often for the first time in years — the chance to experience intimacy without pain. During the drug's action (several months), the pathological reflex fades, and in most patients the effect persists after the toxin wears off.

The procedure takes about 30 minutes and is performed by a physician trained in both sexology and surgery. When indicated, treatment is complemented by work on the psychogenic component and pelvic floor exercises.

Questions & Answers

How safe is this?
Botulinum toxin has been used for decades in neurology, urology and aesthetic medicine. At the doses used, the drug acts locally; the procedure is performed by a physician after examination and ruling out contraindications.
When does it take effect, and how long does it last?
Muscle relaxation develops within 1–2 weeks. The drug's effect lasts several months — during which most patients develop a new, pain-free experience, and repeat injections are often unnecessary.
Will I lose sensation?
No. The drug acts on excessive muscle tone, not on sensory nerve endings.
Is one procedure enough?
For many patients — yes. With longstanding vaginismus, the doctor may recommend a repeat injection along with sexologist support to consolidate the result.
